Reason: It is the same old dCache again where for some reason a directory is created needed for a transfer and the dCache srm forgets to change the ownership to the proper user. This way the directory is owned by root and consequently all transfers to that directory fail. This bug has been in there for years and is still not fixed yet.

Reason:It seems the transfer of this file was never completed. It could be that a prepareToPut was issued, but a transfer never took place. Alternativley (and probably more likely) it could be as a result on srmd being overloaded during this period when FTS resubmissions led to false indications of success. The underlying cause of this has been identified but cannot be corrected in SRM-V1. It is only ever liable to happen when the srm end point is under extreme load.
